  1. ASSUMPTION OF RISK
    I understand that Brazilian Jiu Jitsu is a contact sport involving strenuous physical activity and inherent risks, including but not limited to: sprains, strains, fractures, dislocations, concussions, paralysis, or even death. 

    I voluntarily choose to participate in training, classes, open mats, competitions, and any related activities offered by Solecki Jiu Jitsu Academy, fully understanding and accepting these risks.
  2. RELEASE OF LIABILITY 
    In consideration of being allowed to participate, I hereby release, waive, discharge, and covenant not to sue Solecki Jiu Jitsu Academy, its owners, instructors, employees, agents, members, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss that may occur as a result of my participation.

    This includes claims caused by negligence, to the fullest extent permitted by law.
  3. MEDICAL AUTHORIZATION
    I certify that I am physically fit and have no medical conditions that would prevent my safe participation.

    I authorize Solecki Jiu Jitsu Academy to obtain emergency medical treatment for me if necessary.  I understand that I am responsible for any medical expenses incurred.
  4. RULES AND CONDUCT
    I agree to follow all academy rules and instructor guidance, including:
    -Tapping early to avoid     injury 
    -Maintaining control during sparring
    -Practiving good hygiene
    -Respecting training partners

    I understand that failure to follow rules may result in removal from training.
  5. PHOTOGRAPHY AND MEDIA RELEASE (OPTIONAL)
    I grant permission to Solecki Jiu Jitsu Academy to use photographs or videos of me for promotional purposes, including social media, website, and marketing materials.

    I have read this agreement, fully understand its terms and sign it freely and voluntarily.

    I understand that I am giving up substantial legal rights by signing this document.
